How do I answer the questionnaire ?

• For each question, you may choose between: very rarely, rarely, often or very often.
• You may select only one answer per question. If you skip a question, you will be unable to proceed. Skipped questions are not highlighted (easy to identify). 
• If you leave the questionnaire for more than 15 minutes, all your answers will be lost and you must start again from the beginning.
• When you are done, validate your answers and you will receive a message confirming that your answers have been recorded. The trainer/consultant who provided you with your login ID and password will send you your answers, in accordance with the agreement you made with him/her.

Technical note : We recommend that you use Firefox to answer the questionnaire. This browser is recognized as the fastest and most reliable to date. It is free (download here ), If you use Explorer, it is recommended that you update your browser. In Windows, just launch Windows Update. If you encounter problems with speed while filling out the questionnaire using Explorer, we suggest that you try another computer, or download Firefox.
